The Proactive Health Management Plan uses a fully insured limited benefit supplemental health insurance plan to provide preventive care that creates healthy living habits. Healthy living is just one of the rewards of the plan. PHMP provides an indemnity payment directly to participating employees. These payments should be used to offset medical expenses – but can be used however the participant wants.

Bone Marrow Testing

A bone marrow test is used to examine the fluid and tissue in your employee's marrow. According to the Leukemia & Lymphoma Society, the test helps determine whether cancer or another disease is affecting blood cells or marrow. 

Stress Test (Bicycle or Treadmill)

A stress allows doctors to analyze the level of physical activity your employees are capable of achieving. The test is normally conducted using a treadmill or bicycle. Participating in a stress test lets PHMP know the level of physical activity that's right for your employee.

CA 15-3 (Breast Cancer)

Cancer Antigen 15-3, or CA 15-3, is a test used to monitor breast cancer treatment. The test also helps track breast cancer reocurrence. The CA 15-3 is for advanced breat cancer and is tested by measuring the amount of CA 15-3 in the blood.

CEA (Blood Test for Colon Cancer)

Carcinoembryonic antigen, or CEA, is a blood test used to help diagnose and manage certain types of cancers.

Chest X-Ray

 A chest x-ray can evaluate the lungs, heart, and chest wall of an employee. An x-ray is normally used to diagnose and monitor various lung conditions like pneumonia, emphysema, and cancer. 

PSA (Prostate Cancer)

Prostate-specific antigen, or PSA, is a protein produced by prostate gland cells. The PSA test measures the level of PSA in men's blood. 

Serum Protein Electrophoresis (Myeloma)

A Serum Protein Electrophoresis, or SPEP, test measures specific proteins in the blood to help diagnose diseases. PHMP uses the SPEP test to diagnose an employee's risk of Myeloma. Myeloma is a cancer of the plasma cells. 

Breast Ultrasound

Breast Ultrasound's use sound waves to create computer images of the inside of the breast. Breast ultrasounds can determine if a lump is solid or filled with fluid. Breast ultrasounds are the right choice for employees if they are at high risk for breast cancer, are pregnant, or have increased breast density.

Thermography (Infrared Photo)

Thermography is a test that detects and records temperature changes on the surface of the skin. Thermography can be used to screen for breast cancer using an infrared thermal camera to take photos of the areas exhibiting different temperatures.

Sometimes, professional help is needed for maximum fitness results.  PHMP knows how to cut thorugh the confusion and find the best specialists for your employee's health needs. A health coach cares about your employee's overall health goals such as weight loss or eating better. Unlike nutritionists they cannot prescribe specific meal plans, however, they can motivate employees.

Thorough communication advancing new fitness tips while enocuraging conversations on the topic will help employees reach their goals. Whether overeating or undereating your poor dietary decisions have an effect on your total health. Telemedicine, the use of telecommunication for clinical health care, is one of the advanced features PHMP offers. With telemedicine, employers and employees will no longer need to make late night visits to the emergency room for minor issues. With our limited medical plan, you can save time, money, and stay healthy. The average wait time for an emergency room patient is over 2 hours. In the case of an emergency you should always visit a hospital, but for minor headaches or colds a call to our licensed doctors will be sufficient. Our doctors are on standby 24/7 to help with your medical needs, and they will treat and prescribe your illness. Talk to a doctor anytime, anywhere. Our network of licensed doctors are standing by 24/7 to help. They can diagnose, treat and often prescribe for an array of medical issues right over the phone—even from the comfort of your couch!


Biometric Screening identifies risk factors for illnesses such as diabetes and cardiovascular disease up to ten years before full onset. Our wellness coaches analyze screening data to provide and monitor adaptive strategies and solutions. Biometric Screenings are the building blocks of PHMP's benefits.

A biometric screening is the measurement of physical characteristics such as height, weight, body mass index, blood pressure, blood cholesterol, blood glucose, and aerobic fitness tests that can be taken at the worksite and used as part of a workplace health assessment to benchmark and evaluate changes in employee health status over time. By conducting a biometric screening your company will have a baseline analysis of every employee's health status. 


PHMP contains a Health Risk Assessment (HRA) to determine which aspects of an employee’s well-being require attention. PHMP uses the collected data to customize a program to each individual’s needs.


DNA screening provides information pertaining to diet, nutrition and exercise. Data generated from testing genetic markers in a number of genes can be used to understand how to modify lifestyle and behaviors for optimum wellness.

How Screenings Help Your Diet and Nutrition Plan

Many people use fad diets because they do not have access to what's actually going to help them lose weight or become healthy. We're all accustomed to using Google for all of our problems, but the internet and even a medical label doesn't truly know you. At PHMP we offer DNA screenings to help employees know exactly what they need to be putting or cutting from their diet to live a healthy lifestyle. If your employee wants to fit into "those" jeans, then we can help by looking at their genes. For example, a study found that people with two particular variants of the FGF21 gene were 20 percent more likely to enjoy and seek out sugary substances than others. People call it a "sweet tooth," but using a DNA screening would inform a nutritionist of the gene and allow them to help prevent employees from developing type 2 diabetes, which is often linked to a high-sugar diet.
